West Virginia recorded the most mining fatalities with six — all at coal operations. Texas had four, followed by Missouri with three. Metal/nonmetal operations accounted for 25 of the 33 deaths, with coal operations responsible for the remaining eight.

The industry's fatality increase comes amid a period of increased domestic mineral production demand, raising questions about whether production pressure is affecting safety standards.
